Benazepril is a class of drugs whose main indications are thetreatment of hypertension and heart failure. They exert their hemodynamiceffect mainly by inhibiting the renin-angiotensin system. They also modulatesympathetic nervous system activity and increase prostaglandin synthesis. Theycause mainly vasodilation and mild natriuresis without affecting heart rate andcontractility. Benazepril is anangiotensin-converting enzyme (ACE) inhibitor, is a prodrug which, when hydrolyzedby esterases to its active Benazeprilat, is used to treat hypertension andheart failure, to reduce proteinuria and renal disease in patients withnephropathies and to prevent stroke,myocardial infarction and cardiac death in high-risk patients. Benazeprilcompetitively binds to and inhibits ACE, thereby blocking the conversion ofangiotensin-I to angiotensin-II. This prevents the potent vasoconstrictiveactions of angiotensin-II resulting in vasodilation. Benazepril also decreasesangiotensin-II induced aldosterone secretion by the adrenal cortex, which leadsto an increase in sodium excretion and subsequently increases water outflow.
